Responsibilities

Provides direct and indirect nursing care for the young adult through geriatric age group in an outpatient setting. Functions as an advice and triage nurse by telephone and for walk-in patients. Provides professional leadership, collaboration, respect and assistance to members of the health care team. Provides age-appropriate and culturally sensitive patient and family preventive health education and chronic disease management skills. Five (5) years RN experience in a combination of both inpatient and outpatient work settings.

Current RN license. Current CPR certification

Telephone triage experience.

Recent experience working in a clinic setting within last year or two years. 

Experience providing preventive Health Education and chronic disease management skills in outpatient setting.

Demonstrated skill in verbal and written communication for safe and effective patient care and to meet documentation standards. Demonstrated ability to develop and monitor Quality Improvement activities. trong organizational skills. Baccalaureate in Nursing preferred. Telephone Triage Certification preferred.

Training in Microsoft word, excel and access. Working knowledge of computers.

Ability to communicate effectively with a wide spectrum of the public with diverse ethnic and cultural background.

Ability to communicate effectively in a second language - Spanish, Chinese or Vietnamese desirable.

Minimum of 3-5 years combined experience in healthcare industry 3-5 years of experience with a minimum of 1 year in area of related field Telephone Triage Certification preferred. Training in Microsoft word, excel and access. Working knowledge of computers. Ability to communicate effectively with a wide spectrum of the public with diverse ethnic and cultural background. Ability to communicate effectively in a second language - Spanish, Chinese or Vietnamese desirable. Current RN license. Current CPR certification
